Jose Solarez Scholarship The Jos? Sol?rez scholarship has been established by the Boards of the Arizona Foundation for Behavioral Health and the Arizona Behavioral Health Corporation to recognize and to honor Mr. Sol?rez? life. The scholarship supports an educational opportunity for an individual from the Guadalupe community who seeks to foster his or her educational progress in a recognized postsecondary in Arizona that prepares the applicant for employment in the behavioral health sector.

Minimum requirements are graduation from high school with a GPA of 3.00 or a transcript for any post-secondary education.

A resume or similar form that includes the following information: ? Personal Information ? Educational Information ? Employment History ? Two reference letters from non-family members
Please submit a short statement of your plans after graduation from a college or University. The statement should be no more than 500 words. Please respond to these questions:

What are the ways you would like to use this degree?
